Job Title: Certified Nursing Assistant (CNA)
Location: Fargo, ND

Job Summary:

Centric Solutions is seeking compassionate and reliable Certified Nursing Assistants (CNAs) to join our team. As one of the few CQL-accredited home health agencies in North Dakota, we pride ourselves on delivering high-quality, person-centered care. Our CNAs play a vital role in enhancing the lives of our clients by providing exceptional in-home support.

Responsibilities:

✅ Assist clients with daily living activities, including bathing, dressing, and grooming
✅ Provide mobility assistance, including transferring and repositioning
✅ Perform light housekeeping tasks, such as meal preparation and laundry
✅ Monitor and report changes in clients’ physical or emotional conditions
✅ Offer companionship and emotional support to clients
✅ Follow care plans and work under the supervision of an RN
✅ Maintain accurate documentation of services provided
✅ Ensure compliance with all safety and health regulations

Qualifications:

🔹 Must have a valid Certified Nursing Assistant (CNA) license in North Dakota
🔹 Minimum 6 months to 1 year of experience in home health care or a similar setting (preferred)
🔹 Ability to work independently with minimal supervision
🔹 Excellent communication and interpersonal skills
🔹 Reliable, compassionate, and committed to quality care
🔹 Must pass a background check and drug screening
🔹 CPR and First Aid certification (preferred)
🔹 Valid driver’s license and reliable transportation (preferred for home visits)

Job Title: Registered Nurse (RN)

Location: Fargo, North Dakota

Job Summary:

We are seeking a compassionate, dedicated, and experienced Registered Nurse (RN) to join our team at Centric Solutions. As a vital member of our healthcare team, the RN will be responsible for providing high-quality, personalized care to patients in their homes. The ideal candidate will have a passion for helping others, a strong attention to detail, and the ability to work collaboratively with patients, families, and caregivers.

Key Responsibilities:

  • Conduct thorough patient assessments and create personalized care plans.

  • Administer medications, injections, and treatments as prescribed.

  • Monitor patient progress and report any changes to the care team and physicians.

  • Provide wound care, IV therapy, and other clinical services as required.

  • Educate patients and their families on medical conditions, medications, and care plans.

  • Maintain accurate and up-to-date patient records and documentation.

  • Work closely with caregivers and other healthcare professionals to ensure coordinated and effective care.

  • Ensure a safe and comfortable environment for patients at all times.

  • Follow all infection control policies and maintain a clean and safe working environment.

Qualifications:

  • Active RN license in the state of North Dakota.

  • Minimum 2 years of experience in home health care, hospital, or long-term care settings.

  • Strong knowledge of medical conditions, treatments, and medications.

  • Excellent communication and interpersonal skills, with a compassionate approach to patient care.

  • Ability to work independently, manage time efficiently, and prioritize tasks effectively.

  • Basic Life Support (BLS) certification; Advanced Cardiovascular Life Support (ACLS) is a plus.

  • Ability to provide care across a range of patient needs, from post-surgical recovery to chronic illness management.

Preferred Qualifications:

  • Experience in home health care or palliative care settings.

  • Certification in wound care or other specialties.

  • Experience working with elderly and/or disabled populations.

Physical Requirements:

  • Ability to lift up to 25 pounds and assist patients with mobility needs.

  • Ability to stand, sit, or walk for extended periods of time.

  • Ability to perform tasks that require manual dexterity, such as administering injections and taking vital signs.
